- Book Report Form (These forms are to be turned in with book projects and for the Literary Scholar Award.)
- Literacy Scholar Award Tracking Sheet- This is a copy of the tracking sheet that your student has in their reading folder at school. After your student completes a book project or after they turn in a book report form, they will mark it down on this tracking sheet.
